CHNA 12 Communities:
The Community Health Network Area 12 (CHNA 12) includes following towns and cities:
- Amesbury
- Boxford
- Georgetown
- Groveland
- Haverhill
- Merrimac
- Newbury
- Newburyport
- Rowley
- Salisbury
- West Newbury
CHNA 12 is one of twenty-seven Community Health Network Area local collaborative across the Commonwealth of Massachusetts formed in 1992 by the Massachusetts Department of Public Health: Office of Healthy Communities. CHNA 12, through its collaboration with health and human service providers, community based organizations, faith-based groups, coalitions and taskforces, residents, businesses, community leaders and municipal governments, seeks to improve the health outcomes in the above communities and increase communication and interconnectedness among service providers, consumers, and residents .
Mission:
This health network is a collaborative whose mission is to improve the health status of the community
The network accomplishes this mission through educating the general population through media, providers and consumers.
Governance and Membership :
- Membership is open to all consumers, residents, providers, business, local leaders and municipal governments in the network.
- Members are not categorized by the term, “provider”; more than one member may participate from a single provider or parent company.
- Members are encouraged to reach out to communities and recruit additional members to the CHNA 12 to have a broader representation.
Decision Making & Voting:
- Voting eligibility is accrued and maintained by consistent attendance. To be eligible for voting, a member must have been in attendance, or sent and clearly identified on the attendance sheet a proxy, for at least three meetings in the preceding 12 months period.
- Liaisons representing Massachusetts Department of Public Health and Northeast Center for Healthy Communities serve as resources to CHNA 12 and are non-voting members of the collaborative
- Every effort must be made to reach a consensus in decision making since this is a collaborative.
- Suggestions for any actions may be introduced to the membership from the floor at any meeting.
- Suggestions for action may be submitted to the Steering Committee for review and/or discussion, or for inclusion in agenda materials prior to an at- large meeting.
- Votes are determined by simple majority of all in attendance; and no quorum is necessary.
Oganizational Structure:
The CHNA 12 steering committee is the governing body of the CHNA 12. The Steering Committee has the executive powers in the matters of the CHNA 12. The Steering Committee coordinates CHNA 12 activities in a timely manner, lends consistent volunteer leadership and record keeping, and services as a clearing house for the membership as it relates to activities, governance and CHNA 12 priorities. The Steering Committee is comprised of a minimum seven and a maximum 11 members from the network and a Fiscal Conduit Designee. The Steering Committee members serve two-year terms, are elected in June, and may serve no more than two consecutive two-year terms. However, for all intention and purpose of carrying out the CHNA 12 functions, exceptions may be made to this provision, if it deemed necessary.
All Steering Committee meetings are open to all members of CHNA 12 and are held during the week following the at-large meetings. The CHNA 12 general meetings are held on the second Thursday of each month, from 3:00 to 5:00 PM at Amesbury Health Center, 24 Morrill Place, Amesbury. The Steering Committee Meeting day, date, time and place are determined at the beginning of the fiscal year to accommodate the steering committee member's schedules.
Steering Committee Responsibilities:
- The Steering Committee is the governing body of the CHNA 12. The Steering Committee has executive and administrative oversight of the functions of the CHNA 12.
- Research and make recommendations to the full membership on requests from the membership.
- Provides administrative support for the CHNA 12. All members are responsible for aiding in the administrative activities, such as minute taking, as is necessary. Assignments may be made, or volunteers may come forward.
- Attend all Steering Committee meetings and participate in all Steering Committee activities that relate to CHNA 12.
- Research funding opportunities for CHNA 12.
- Oversee CHNA 12 fiscal matters including budget and CHNA 12 related activities of the fiscal conduit.
- Establish ad hoc committees as needed to address specific needs of the CHNA 12.
Fiscal Conduit:
The CHNA 12 has chosen a fiscal agency with the intention of having its financial transactions administered by a nonprofit entity that does business with the Commonwealth of Massachusetts.
- Legally responsible for the financial management of the CHNA 12 funds. The Fiscal Conduit must maintain a separate bank account for CHNA 12.
- Administer CHNA12 funding from DPH and other sources on behalf of CHNA 12.
- Accounts payable and receivable.
- Submit monthly financial report to Steering Committee.
- Make all payments on time.
- No payment shall be made without an approved payment voucher signed by the CHNA 12 Treasurer.
- Administer CHNA 12 mini-grants on behalf of CHNA 12.
- May designate a representative to the CHNA 12 steering Committee. The Designee will be an ad-hoc member and has no voting privileges. The designee cannot be an office holder of the CHNA 12.
- Comply with all reporting requirements of the funding sources.
Community Projects Funding:
CHNA 12 may be able to make funding available for special one-time projects during the fiscal year, July 1 st to June 30 th . When funding is available, the community projects must meet following criteria:
- All projects submitted for funding must adhere to the mission of the CHNA12.
- Must directly benefit the communities in CHNA 12.
- Must address the major themes selected by CHNA 12 for the funding year.
- Must have clear goals and the potential for long-term impact.
- Funds should not be used for registration fees to participate in health fairs, conferences and other events.
- Funds cannot be used for the purchase of equipment, agency personnel and stipends for participants.
Proposal Requirements:
A brief project narrative, no more than four pages long, should be submitted to CHNA 12 steering committee outlining the following:
- Project description
- Project goals
- Target population
- Program activities
- Measurable outcome
- Budget and budget narrative
- Project contact
All proposals must be typewritten and double-spaced. The proposal should be electronically submitted to the Chairperson of the CHNA12 and a hard copy mailed to:

All proposals will be reviewed by the CHNA 12 Steering Committee and approved by the CHNA 12 Steering Committee for funding. Once the Steering Committee approves for funding, the recommendations are brought to the full CHNA 12 membership for the final vote at the next regular meeting.
All funded agencies are required to comply with the following during the funding period:
- Attend CHNA 12 meetings.
- Report to the full CHNA 12 about the progress of the project at their regular monthly meetings.
- Submit final report to the CHNA 12 Steering Committee at the end of the Project or by the end of the fiscal year.
